In-person new patients should complete the New Patient Intake Form (2 pages),
and then fax it to Dr. Pastor ( at Fax number 609-710-7666 ) in advance of your first appointment.

New patients should also print and complete the following rating scales, including the Patient Health Questionnaire form (PHQ-9), and the HERO wellness scale, and the GAD-7 — and bring these 3 completed forms to your appointment.
